We will be accepting Request for Applications (RFA) for the Fiscal Year 2012-2013 shortly. Please ccheck back to this site as we will post the required forms. Grant Awards for Fiscal Year 2012-2013 will be announced in April 2012. These grants will fund breast cancer treatment, screening and education programs in the following parishes only: Jefferson, Orleans, Plaquemines, St. Bernard, St. Charles, St. Tammany, St. John the Baptist and Washington. East Jefferson General Hospital Foundation - $5,000
Susan G. Komen for the Cure New Orleans Affiliate small grant award to fund the purchase of breast cancer specific educational materials for the new Outpatient Oncology Infusion Center, Cancer Resource Library.

St. Thomas Community Health Center - $ 228,727.50 - April 2011

St. Thomas Community Health Center is a non-profit primary health care clinic serving the uninsured & underinsured of the New Orleans region. Since 2005, the clinic has provided breast cancer early detection for women living in poverty. This grant will assure the continuation of this community based effort. The grant will provide for mammography in the 40-49 year age group, a group not funded by the large Federal Grant that provides mammography for uninsured women. In addition, the grant will fund diagnostic ultrasounds & in further investigating problems seen on mammography imaging. Funding will also be used to maintain the remote reading equipment to allow for reading of the imaging by the radiologist off site within 24 hours. Lastly, the grant will fund critical personnel. These include the Diagnostic Care Coordinator who facilitates the diagnostic test scheduling at outside facilities, maintains contact with the patient & is the fail safe monitor for all women in the program, & the Nurse Practitioner who provides diagnostic & screening exams, patient education & case management to assure that all patients receive quality evidence-based care.

LSUHSC School of Public Health - $ 137,322.50 - April 2011

The Breast Center is an innovative collaborative of the LSUHSC School of Public Health Louisiana Breast and Cervical Health Program (LBCHP), the LSU Interim Public Hospital, and the Susan G. Komen for the Cure New Orleans Affiliate. The Center decreases waiting times for diagnosis and treatment of breast cancer for uninsured women in the New Orleans area. Funding from Komen is allowing continuation of services, including education, outreach, transportation assistance to appointments at the Center, breast exams, mammography, diagnostic testing by specialist physicians, case management and navigation, and access to treatment. The Center boasts a highly skilled, multi-disciplinary team, many of whom have been with the program since its inception and through Hurricane Katrina.

 

Leonard C. Thomas HOS Foundation - $ 31,600 - April 2011

The Leonard C. Thomas HOS Foundation's mission is to enhance the quality of life of cancer patients and their families throughout their cancer experience. In keeping with our mission, our vision is to become a model, first class foundation capable of meeting the needs of cancer patients and their families throughout the Greater New Orleans community. Our patient oriented approach and the ability to provide quality services completely free of charge regardless of economic status help to make us a unique non-profit cancer organization. Our programs include Lymphedema Screening and Therapy, Genetic Counseling, Yoga, Mindful Meditation and Psychosocial Services which includes a part time social worker to help patients find appropriate programs. Patients will be evaluated quarterly and a summary report will be sent to Komen at six months and at the end of the grant period. We would like to incorporate transportation for patients along with lymphedema and psychosocial services as these programs definitely impact their quality of life.

West Jefferson Medical Center Outpatient Rehabilitation Department - $ 30,100 - April 2011

Lymphedema is a non-curable condition in which fluid accumulates in the arm, back and/or chest causing uncomfortable swelling & decreased functional use of the affected arm with daily tasks. The grant funds will be available to breast cancer survivors to aid in purchasing compression materials necessary to reduce and/or stabilize the fluid build up associated with lymphedema. Our department offers 2 programs to address the physical, emotional & social complications & side effects of treatment, the Recovery in Motion Program & the Lymphedema Management & Education Program. Early entrance into these specific physical rehabilitation programs will result in a shorter length of stay for treatment, increased probability of symptom reduction, increased independence with activities of daily living & improved quality of life. To facilitate early detection of physical deficits & to promote early entrance in the programs, consistent & ongoing community & medical staff education is imperative. Grant funds will be directly utilized to support education of the professional treating staff & to promote awareness of wellness & lymphedema management before, during & after breast cancer treatment.

West Jefferson Medial Center Women's Imaging & Breast Care Center - $ 26,250 - April 2011

In 2010, the Women's Imaging & Breast Care Center of West Jefferson Medical Center successfully implemented the Susan G. Komen Breast Care Grant at the facility. This grant provided services for women that met the required guidelines of the program that live in the Greater New Orleans area. To date, the grant has provided over 128 screening mammograms, 20 diagnostic mammograms and 27 Breast Ultrasound exams to minority women and the underserved women in the community. The program criteria includes:* Free Mammography and Ultrasound services to the underserved, indigent, or minority women in the community. *The women must be at least 40 years old or have a strong family history of breast cancer. *Have no medical insurance. *Living within or below the 165% of the 2011 income eligibility standards of the poverty guidelines. The 2011 Komen Grant will provide the services of Screening Mammograms and if needed, follow-up Diagnostic Mammograms and Breast Ultrasound procedures. Breast cancer is the most commonly diagnosed cancer of women living in the US. The National Cancer Institute estimates that a woman in the US has a 1 in 8 chance of developing breast cancer in her lifetime.

St. Charles Community Health Center, Inc. - $ 26,000 - April 2011

St. Charles Community Health Center seeks to support its breast health education outreach by providing breast screening for all women in the communities we serve with special emphasis on the uninsured/under insured low-income and/or minority women. Its goal is to reduce breast cancer mortality rates by increasing knowledge and compliance with breast screening guidelines, resulting in earlier detection. Program objective are to provide outreach education to at least 3,000 women, to provide screening mammograms to at least 500 women, and follow-up diagnostic mammograms to as many as 50 women. Each program participant will be taught breast self-exam techniques using anatomically correct models and will receive a clinical breast exam from a primary care provider of St. Charles Community Health Center. Educational programs will be conducted throughout the community providing instructions on breast self-exams and the importance of clinical breast exams and mammograms. The Health Center will also partner with other health care providers in the community to promote breast health care whenever possible.

CAGNO Cancer Association of Greater New Orleans - $ 20,000 - April 2011

Middle class and working class people in South Louisiana are feeling the progress of recovery from disasters. Unfortunately, our recovery comes at a time of national recession and loss of jobs. The current economy is a real threat to the well being of breast cancer patients. At a time when a patient's energies should be directed toward fighting breast cancer, many patients must deal with loss of income, insurance, and the high cost of treatment. Our Access database tracks patients, services, demographics, outcomes and costs.
Susan G. Komen for the Cure and CAGNO have partnered in the past, and together we were able to help women on their journey from breast cancer patient to breast cancer survivor. We propose to offer to breast cancer patients temporary financial aid such as: prescription outpatient medications, insurance premiums, COBRA fees, co-pays, and cancer-related medical bills. Recipients of this service will be patients who do not qualify for government assistance, but cannot afford proper medical care while they are out of work. Patients will be primarily women who have never had to ask for anyone's help before. Financial aid can lead to health and independence.

LSUHSC School of Public Health - $167,270, April 2010

This grant will provide funding for the New Orleans Breast Programs by LSUHSC which will allow for an Advanced Practice Nurse and a Diagnostic Care Coordinator. The Mammo Machine Maintenance Contract at ST. Thomas will be funded also. Komen is allowing the re-creation and continuation of services, including education, outreach, transportation, breast exams, mammography, diagnostic testing, case management and navigation, and access to treatment. The center has a highly-skilled, multi-disciplinary team, many of whom have been with the program since inception.

Leonard C Thomas HOS Foundation - $54,740, April 2010

This grant entitled "Enhancing the Quality of Life through Programs for Breast Cancer Patients and their Caregivers" will provide lymphedema therapy supplies, patient care costs associated with lymphedema therapy and screenings. Leonard C. Thomas Foundation's mission is to enhance the quality of life of cancer patients and their families throughout their cancer experience. Their vision is to become a model, first class foundation capable of meeting the needs of cancer patients and their families throughout the Greater New Orleans community. They have a patient oriented approach with the ability to provide quality services completely free of charge regardless of economic status.

Cancer Association of Greater New Orleans - $10,000, April 2010

This grant entitled "Breast Cancer: Hope and Help" will provide financial assistance for qualified breast cancer patients with medications, medical bills, insurance premiums, COBRA fees, co-pays and breast cancer related medical bills. Susan G. Komen and CAGNO have partnered in the past, and together we were able to help women on their journey from breast cancer patient to breast cancer survivor. Recipients of these services will be low to medium income patients who do not qualify for government assistance, but who cannot afford proper medical care while they are out of work. Financial aid, now when they need it most, can lead to health and independence.

St. Thomas Community Health Center - $196,542, April 2010

This grant entitled "St. Thomas Community Health Center Breast Cancer Screening and Early Detection Program" will fund radiology patient care costs for mammography and ultrasounds. This grant will continue to assist those who are uninsured and underinsured. The St. Thomas Community Health Care Center has been one of the largest providers of primary care for the uninsured in the New Orleans region for the last 20 years. Since the Hurricanes of 2005, Komen has been critical in the development and continuation of these breast cancer early detection services in the this community setting. The funding will provide for mammograms in the 40-49 year old age group, a group not funded by federally sponsored programs, and it will fund ultrasounds to be done on site at the clinic. Providing this early detection program in a trusted , easily accessible community setting for women who face dire socio-economic challenges has played, and will continue to play, an important role in improving survival.

West Jefferson Medical Center Women's Imaging and Breast Care Center- $25,125 , April 2010

This grant will assist with patient care costs, screening and diagnostic mammograms for the underserved, indigent or minority women in the community. If needed, there will be follow up Diagnostic Mammogram and Breast Ultrasound procedures. Women must be 40 years of age or have a family history of breast cancer, have no medical insurance and living within or below the 165% of the 2010 income eligibility standards of the poverty guidelines.

West Jefferson Medical Center Outpatient Rehabilitation Department- $24,000, April 2010

This grant will support the Breast Cancer Rehabilitation and Lymphedema Management and Educational Program. Komen funding will be available for the purchase of compression materials neccessary to reduce and/or stabilize the fluid build up associated with LE. Early detectionof symptoms and entrance into the Recovery in Motion program result in a shorter sourse of treatment, increased probability of symptom reduction, increased independence with daily tasks and better quality of life. Funding will also support education and community awareness about the programs.

St. Charles Community Health Center- $36,500, April 2010

The St. Charles Community Healthe Center seeks to support education, outreach and breast screening for all women in the communitywith special emphasis on the uninsured/underinsured low-income and/or minority women. Its goal is to reduce breast cancer mortality rates by increasing knowledge and compliance with breast screening guidelines, resulting in earlier detection. Program objectives are to provide outreach education, screening mammograms and follow up diagnostic mammograms. Each participant will be taught breast self exam techniques. Assistance with patient care costs is available to the recipients for these services.

The foundation will use this years grant funds to provide Enhancing the Quality of Life Programs for Breast Cancer Patients and Their Caregivers. Lymphedema supplies and patient costs associated with screening and therapy (administered by a licensed occupational therapist), yoga (offered on a weekly basis in three locations) and psychological counseling (provided by a licensed counselor for patients and family members) will also be provided.

Cancer Association of Greater New Orleans - $20,000, April 2009 -

CAGNO Their program Breast Cancer: From Patient to Survivor offers breast cancer patients temporary financial aid for prescription outpatient medications, insurance premiums, COBRA fees, co-pays, and breast cancer-related medical bills. Recipients of the services are low-to-medium income patients who do not qualify for government assistance, and who cannot afford proper medical care while they are out of work – primarily working women and mothers. This financial aid, when they need it most, can lead to health and independence.

St. Thomas Community Health Center - $80,330, April 2009 -

The funding will provide mammography and breast cancer education for uninsured and underinsured women in Orleans and surrounding parishes. An expansion of an already existing program, this year’s funding will provide for the continued outreach program managed by a lay health educator, and equipment for telemammography (an exciting new electronic service with the capacity to send mammograms offsite within 24-48 hours). Radiology Professional fees-mammography & ultrasounds and Remote Reading maintenance will also be provided.

West Jefferson Medical Center Outpatient Rehab Dept. - $25,000, April 2009 -

The established Lymphedema Management and Education Program at WJMC’s Outpatient Rehab will use grant funds to reinforce the importance of early detection and treatment of breast cancer related lymphedema symptoms and provide financial assistance to breast cancer survivors in need of compression materials. During this grant period, funding will be used to increase awareness of breast cancer lymphedema through community, medical and professional staff education in the form of in-service, marketing calls, and tuition for certification course training for an outpatient staff therapist.

Edna B. and Joyce Fay Washington Breast Cancer Foundation, Inc. - $1,440, March 2008 –

This foundation has provided support services to medically underserved women in the greater New Orleans community since October, 1993. The program is designed to provide needed support and education for good breast health care from “one time, topic specific” educational seminars. This grant will fund a breast self-exam model set and supplies.

The foundation will use the grant funds to provide quality of life programs for the benefit of breast cancer patients, including lymphedema therapy (administered by a licensed occupational therapist), yoga (offered on a weekly basis in three locations) and counseling (provided by a licensed counselor for patients and family members).

LSUHSC Interim Hospital at New Orleans (University Hospital Campus) - $23,885, March 2008

The rehabilitation department at LSUHSC Interim Hospital has a breast cancer program for patients currently undergoing treatment, as well as those experiencing difficulties secondary to breast cancer lymphedema. The program is available to patients in Orleans and surrounding parishes, regardless of their ability to pay, and offers inpatient pre-operative and post-operative rehabilitation. The grant will fund the education of two replacement therapists, equipment and supplies.

LSUHSC School of Public Health (Lord & Taylor Campus; later University Hospital or new facility) - $143,524, March 2008 –

The devastation of Hurricane Katrina in New Orleans resulted in the loss of the Breast Center at the Medical Center of Louisiana at New Orleans, an innovative collaborative of the LUSHSC School of Public Health Louisiana Breast and Cervical Health Program and the Susan G. Komen for the Cure New Orleans affiliate.
